The Details of the 2nd prize – Lasik surgery:

One prize certificate valid for laser vision correction (LASIK) surgery, which consists of using laser technology to reshape the cornea (the “Procedure”) as determined in the sole and absolute discretion of L. Scott Feiler, M.D. up to a $4,000.00 value.  Procedure will be performed on the winner (or winner’s eligible designee as described herein) at the sole and absolute discretion of L. Scott Feiler, M.D. and shall consist of laser vision correction for nearsightedness or nearsightedness with astigmatism, with limit on prescription/treatment as set forth in the LASIK rules and regulations, on one or both eyes for ONE qualified patient.  The Approximate Retail Value ("ARV") of the Raffle Prize is $4000 for a bilateral (laser vision correction on two eyes) or $2,000 if winner elects monocular (laser vision correction on one eye).

This prize is not redeemable for cash and may not be combined with any other offers or medical/vision discounts. Odds of winning the raffle depend on the number of raffle tickets sold

The LASIK procedure must be performed within six months by Jan. 31, 2017. Scheduling is subject to availability. The procedure to be performed at L. Scott Feiler, M.D. office located at 2894 El Camino Real, Redwood City, CA 94061

Rules and Restrictions:

Must be 18 years or older to participate

LASIK procedure for this contest refers to a laser vision for nearsightedness or nearsightedness with astigmatism on one or both eyes for ONE qualified patient

Treatment for this procedure will be limited to myopic (nearsighted) prescriptions only, not to exceed 5 diopters of nearsightedness and 2 diopters of astigmatism for a qualified patient

Procedure includes evaluation to determine candidacy, LASIK procedure on one or both eyes, and post-operative care.

Post-operative care refers to visits at 1 day, 1 month, 3 months and 6 months following the procedure to monitor results.

Procedure does not include annual eye health examinations or medical visits for eye care not related to the LASIK procedure

Qualification for LASIK procedure to be determined by a comprehensive, dilated, eye health examination by L. Scott Feiler, M.D.  and is subject to review by the LASIK surgeon performing the procedure.  There is no additional fee for the evaluation.

Procedure may be performed on one or both eyes; if winner opts for monocular LASIK (procedure on one eye), he/she may not transfer remaining procedure to another person.

However, winner may transfer entire procedure to another qualified person (to be determined by evaluation of L. Scott Feiler, M.D. and review of LASIK surgeon) of his/her choosing.

Does not include contact lens fitting/trial (up to $279.00) if patient chooses a monovision (one eye for distance, one eye for near) correction.  Contact lens monovision trial period is required for patients desiring monovision treatment

Procedure includes LASIK enhancement (retreatment) for one year following procedure

Eligibility for enhancement will be determined by an eye health examination and will only be performed if it is deemed necessary and safe for the patient to achieve desired results.

Enhancements beyond one year will be subject to a surgical fee.

Enhancements are NOT included for patients who choose monovision treatment and desire monovision reversal.  Patients who choose monovision reversal are subject to evaluation by surgeon to determine if reversal procedure is reasonable and safe to proceed.  Fee for monovisoin reversal is $750.00 during the first year following the initial procedure.

After the first year, ANY LASIK procedure performed will be subject to a surgical fee to be determined by the laser facility

There is no guarantee of LASIK results

Winner/patient must sign an informed consent provided by the laser facility and agree to accept and follow all eye care instructions set forth in the agreement

Patient assumes responsibility for any consequences resulting from failure to follow post-operative instructions as dictated by L. Scott Feiler, M.D.

Raffle Prize winner (and designee, if Raffle Prize certificate is transferred to an eligible patient) must sign a liability release, agreeing to release and hold harmless L. Scott Feiler, M.D., their respective parent, subsidiary and affiliated entities, their promotional partners and advertising agencies, and their respective officers, shareholders, directors, employees, agents and representatives and all of their successors and assigns from and against any and all claims or liability arising directly or indirectly from the Raffle Prize and participation in the Raffle.

Does not apply to complex LASIK procedures and excludes the following:

Patients who have had previous eye surgery

History of eye disease, i.e. thin corneas, keratoconus, Fuch's Disease, excessive dry eye disease

History of systemic or autoimmune disease, i.e. rheumatoid arthritis, lupus, diabetes or other disease that may affect healing or surgical outcome

Greater than 5 diopters of myopia (nearsightedness)

Greater than 2 diopters of astigmatism with or without myopia
